Transcript Release Form

Welcome to Mid-America Christian University!

MACU will admit students with unofficial transcripts, but all official transcripts are required to be on file by the end of the first term. By submitting this form, we will be able to assist the student with ordering their official transcripts.

Please note: Official transcripts will be ordered after the start of the first course. It is the student's responsibility to provide all unofficial transcripts during the enrollment process. Unofficial transcripts are acceptable for admission. If you need assistance in ordering your unofficial transcripts, please contact your Enrollment Counselor at 888.888.2341.

MACU will make every attempt to request the official transcript on the student's behalf. Some schools do not allow third party ordering and the student must order the transcript. If the student has a hold on their account at the school, we will not be able to request the transcript and it will be the student's responsibility. We will be in contact if there is further information needed.

Form Instructions

Please complete the information below for all colleges/universities that you have attended or were enrolled in for credit. This signed form will grant MACU permission to order your transcripts. Please include all of your previous educational history when applying for admission to MACU.

Once all of the school information has been entered there will be a signature page that you will need to complete in order to submit the form for verification.

There will also be an additional email sent to the email address provided on the signature page. Please click on that email to verify the signature to complete the form.
